Today, most health care providers would likely agree that the old paradigm in which a doctor reels off instructions to a mostly passive patient falls somewhere toward the bottom of the effectiveness scale. In the dawning of the pay-for-performance era, engaging patients in self-management is widely considered a more effective way to improve health and outcomes. Sometimes called Collaborative Self-Management Support (CSMS), this approach has shown promising results in both Improvement Collaboratives focused on chronic care and in some RCT trials. But finding the resources to implement CSMS is a cited as a major barrier to implementing CSMS in the office practice setting. Where can a busy office practice find the time? The extra staff? The training? 

In this one-hour call, we heard from experts who have been working on this critical question in a variety of health care settings, including:

  • Austin Chang, Analyst at Cambridge Health Alliance; a large integrated delivery system serving a safety net population.
  • Greg Pawlson, MD, MPH, Executive Vice President of NCQA, a leading evaluator of health care services including HEDIS® clinical performance measures.
  • David Swieskowski, MD, MBA, Vice President for Quality, Mercy Clinics, Inc. a network of 140 physicians in a 100% fee-for-service environment.
  • Laurel Simmons, SM, Institute for Healthcare Improvement, Deputy Director for New Health Partnerships, an initiative funded by RWJF to refine and spread viable models for CSMS in the ambulatory care setting.
